The Integrated Care System (ICS) strategy
document (Appendix A) submitted as part of the NHS Long Term Plan
(LTP) submissions contained the ICS Population Health Plan, aimed
at improving health and wellbeing outcomes of our communities. A
system wide approach to develop the Population Health
Implementation Plan is under way. This work is being managed
through the ICS Population Health Steering Group.
